Explore how nursing homes are embedding residents' goals, preferences, and priorities (GPP) into care planning and daily care.

Laci Cornelison and Addison Van Zutphen, Moving Forward Coalition's partners at Kansas State University, will share emerging lessons from the Goals, Preferences, and Priorities initiative, which explores how nursing homes can embed residents' goals, preferences, and priorities, or what matters to them, into care planning, daily care, and quality improvement efforts.

Participants will learn:

  • - How the Goals, Preferences, and Priorities initiative helps nursing homes identify and incorporate what matters to residents into care planning and care delivery
  • Key lessons emerging from implementation across participating nursing homes
  • Considerations for integrating residents' preferences into everyday nursing home practice
