Session 2 : New paradigm to improve equitable home and community-based palliative care.
Speakers:
Connie Dahlin (left) and Christine McMichael (right), will build on key implementation elements shared by Transforming Care Partners in Session 1, to collectively envision a Massachusetts community based palliative care (CBPC) program that is highly responsive to the needs of our patients and their families within across diverse communities.
Constance Dahlin, MSN, ANP-BC, ACHPN, FPCN, FAAN, Consultant to Center to Advance Palliative Care (CAPC) will-
- Set out a foundational framework and explore Primary and Specialty Palliative Care (see CAPC report below)
- Update us on the CAPC Serious Illness Scorecard, How does Massachusetts rate, why, and how that impacts CBPC.
Christine McMichael, Executive Directive, Hospice & Palliative Care Federation of MA, will-
- Update us on the National Alliance for Care at Home, a historic merger of the National Association for Home Care & Hospice and National Hospice and Palliative Care Organization;
- Offer insights into how the newly combined organization can improve the continuum of care at home.
